The Department of Education is currently updating all computer systems in schools, requiring additional information . Students have ALL been issued with a "Student Information Update" form during roll call and were asked for their parents/carers to complete and return the form to school at the earliest convenience please. The form is also attached below.
The "Student Information Update" form has been reviewed in line with Department Policy to include the following: Medicare information, Permission to Publish, Religious Instruction and Student's mobile phone number.
Please take the time to complete this form so your child's school records are current and up to date, this is particularly important regarding your child's Medicare information.
Medical forms are available from Admin and can be collected by your child or posted home if preferred.
